The Key Biomechanical Difference
The fundamental difference between treadmill and outdoor running is the source of propulsion. Outdoors, you propel yourself forward relative to a stationary ground — your leg pushes backward against the road and the road pushes you forward. On a treadmill, the belt moves backward beneath you — you primarily lift your feet and place them in the path of the belt, rather than actively driving backward.
The practical consequence: treadmill running reduces the demand on the hamstrings and glutes (the muscles responsible for hip extension and push-off), while outdoor running engages them more fully. Research consistently shows greater hamstring activation in outdoor running at the same pace, and many runners who train exclusively on treadmills find outdoor running harder when they race.

The 1% Incline Rule
The most well-known practical implication of treadmill biomechanics: running at 0% incline on a treadmill is slightly easier than running at the equivalent pace outdoors, because the moving belt provides a small propulsive assist. Research by Jones and Doust (1996) found that setting a treadmill to 1% incline approximates the energy cost of outdoor running at the same speed, primarily by compensating for the absence of air resistance.
For training purposes: set treadmill incline to 1% for easy and moderate runs to better match outdoor training stimulus. For race-specific tempo runs or intervals where you are targeting outdoor race pace, use 1–2% incline to account for both belt assist and the absence of wind.

Calorie Burn Comparison
At 0% incline, treadmill running burns approximately 5–10% fewer calories than outdoor running at the same displayed pace, due to the belt assist reducing net propulsive work. At 1% incline, the difference largely disappears. At higher inclines (3–5%), treadmill running burns significantly more calories than flat outdoor running at the same pace.
