How Cortisol Promotes Abdominal Fat Storage
Cortisol is released by the adrenal glands in response to stress as part of the body's normal "fight or flight" response. In short bursts, this is adaptive — cortisol mobilises energy for immediate demands. The problem arises with chronic stress, where cortisol remains persistently elevated rather than spiking and returning to baseline.
Fat cells in the abdominal region have a particularly high density of cortisol receptors compared to fat elsewhere in the body, which is why chronically elevated cortisol is specifically linked to increased visceral (abdominal) fat storage rather than uniform fat gain. Research measuring cortisol reactivity has found that individuals with higher cortisol responses to repeated stress carry more abdominal fat even at similar overall body weight compared to those with lower cortisol reactivity.

The Stress-Sleep-Weight Cycle
Stress, sleep, and weight interact in a self-reinforcing cycle rather than three independent factors. Chronic stress elevates cortisol, which disrupts sleep onset and quality — and poor sleep, in turn, further elevates cortisol the following day (see our Sleep and Weight Loss guide for the hormonal detail). Simultaneously, elevated cortisol and poor sleep both independently increase appetite and cravings for high-calorie, high-carbohydrate "comfort" foods, compounding the difficulty of maintaining a calorie deficit.
